The sphenomandibular ligament is separated from the neck of the mandible below lateral pterygoid by the maxillary artery and from the ramus of the mandible by the inferior alveolar vessels and nerve and a parotid lobule.

It is formed by a confluence of the veins of the pterygoid plexus, and passes backward between the sphenomandibular ligament and the neck of the mandible, and unites with the superficial temporal vein to form the retromandibular vein.

The intervening part of the cartilage disappears; the portion immediately adjacent to the malleus is replaced by fibrous membrane, which constitutes the sphenomandibular ligament, while from the connective tissue covering the remainder of the cartilage the greater part of the mandible is ossified.

From the proximal end of each cartilage the malleus and incus, two of the bones of the middle ear, are developed; the next succeeding portion, as far as the lingula, is replaced by fibrous tissue, which persists to form the sphenomandibular ligament.

Mylohyoid : by its own nerve, a branch of the inferior alveolar ( from the mandibular division of trigemminal nerve ), which arises just before the parent nerve enters the mandibular foramen, pierces the sphenomandibular ligament, and runs forward on the inferior surface of the mylohyoid, supplying it and the anterior belly of the digastric.

The margin of this opening is irregular; it presents in front a prominent ridge, surmounted by a sharp spine, the lingula mandibulae, which gives attachment to the sphenomandibular ligament; at its lower and back part is a notch from which the mylohyoid groove runs obliquely downward and forward, and lodges the mylohyoid vessels and nerve.
